Job Overview
In this exciting position, you will be responsible for delivering high-quality psychiatric care in an outpatient setting. You will collaborate with multidisciplinary teams to develop individualized treatment plans, conduct thorough evaluations, and provide ongoing support to patients facing mental health challenges. Your contributions will be vital in enhancing patient outcomes and promoting overall community well-being.
What you’ll do
· Provides psychiatric evaluation and ongoing psychiatric care of homeless clients by performing mental status examination, diagnostic assessments, supportive therapy, consultations, and pharmacological management.
· Provides accurate, timely documentation of assessment and care provided to clients.
· Works collaboratively with other physicians, nurses and staff.
· Works closely with behavioral health counselors to assure continuity of care.
· Requests and interprets laboratory evaluations.
· Makes medical referrals/warm handoffs when indicated.
· Completes external reports (probate, disability) as needed.
· Manages prescriptions, administration of medications, and education for clients and thoroughly documents everything in the Electronic Medical Record.
Basic qualifications
- Active Nurse Practitioner license with a specialization in Psychiatric Mental Health Nursing.
- Strong knowledge of psychopharmacology and behavioral health principles.
Preferred qualifications
- Experience with electronic medical record (EMR) systems such as Epic or Cerner.
- Proficiency in conducting telehealth sessions for remote patient care.
- Familiarity with CPT coding, ICD-9, and ICD-10 for accurate billing processes.
- Background in critical care or emergency medicine is advantageous.
- Experience working with diverse populations, including children and seniors.

Work Location: In person
The Behavioral Health Counselor is a critical part of the primary care team in our Health Care for the Homeless Program. The Behavioral Health Counselor assesses patient needs, develops treatment plans, provides outpatient individual and group therapy, monitors progress in treatment, provides psychoeducation, and collaborates with medical and dental departments.
Responsibilities:
· Actively participate as a member of the integrated primary care and behavioral health team.
· Provide behavioral health screening and assessments to patients experiencing homelessness at assigned NeighborHub Health Primary Care Centers.
· Ability to conduct biopsychosocial assessments and monitor ongoing treatment planning needs.
· Provide counseling and brief intervention activities for behavioral health and substance abuse disorders.
· Participate in Quality Improvement activities.
· Work alongside physicians and offer behavioral health advice/intervention when needed.
· Make necessary referrals and discuss cases with clinicians.
· Demonstrate ability to document within EMR system.
· Make referrals to ongoing sources of care for identified problems.
· Other responsibilities as assigned.
Knowledge and Abilities Required:
· Ability to work effectively with teams and a diverse array of professionals and agencies.
· Knowledgeable about the region’s mental health and substance use treatment system and resources.
· Cultural sensitivity and willingness to work with patients experiencing homelessness and patients with HIV

This position is eligible for the National Health Service Corps Loan Repayment Program
Position Requirements:
Education: Master’s in Social Work or Clinical Mental Health Counseling from a CACREP accredited university
Professional License Requirements:
· Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) in the State of Ohio.
